Si solicita informaci\u00f3n de un tratamiento en concreto, s\u00f3lo la informaci\u00f3n de \u00e9ste. Si lo solicita por tel\u00e9fono se le indicar\u00e1 que lo haga por escrito y se le informar\u00e1 de c\u00f3mo puede hacerlo y la direcci\u00f3n a la que tiene que enviarlo.
